RE: CRITICAL INFORMATION FOR APPLICATION PROCESS
This Memo is to inform you that there are two areas that require your immediate attention.
1. IMMUNIZATION INFORMATION REQUIRED PRIOR TO APPLICATION TO UPPER DIVISION
Documentation of current immunization status must be on file with the School of Nursing in order for your application to be considered for admission to upper-division nursing. As this is a process that may take several months to complete, you must initiate the process NOW, particularly if you are making application in Fall 2009 for the Spring 2010 class. Incomplete files will not be considered for admission to upper-division. The most efficient way to accomplish this process is to go through MTSU Health Services (898-2988).
The following immunizations/titers are required:
Measles (Rubeola) with documented positive titer
Mumps with documented positive titer
Rubella with documented positive titer
Varicella with documented positive titer
Hepatitis B Series with documented positive titer (series must at least be initiated if time does not permit completion with titer results). If you opt not to receive this immunization, you must come to the School of Nursing to sign a waiver form.
If any initial titer indicates lack of immunity, an immunization booster must be given with follow-up titer. If second titer is non-reactive or negative, you must provide documentation of these results.
TB skin test – initial skin test must be applied using a two-step process. If there is a history of a positive TB skin test, you must have a chest x-ray. If the chest x-ray is positive, you must be treated. Documentation must be provided.
2. COURSES REQUIRED PRIOR TO APPLICATION TO UPPER DIVISION
All general education and pre-nursing requirements must be completed prior to entering upper-division nursing. These are the courses that must be completed prior to enrolling in upper-division nursing:
