The
following forms are for requesting a facility in Murphy Center.
- The
Murphy Center Complex Request Form is
the initial request form for rental of an MCC facility. This form
is a REQUEST ONLY. Email confirmation from the Murphy Center Complex
Office will be sent to you if your event may be scheduled as requested.
- Application
for Use of Facilities Form* must be completed, signed and
returned after you have received email confirmation. This form
must be received by the Murphy Center Complex Office five business
days BEFORE the event.
